Why would a generic drug be considered Tier 4 Non-preferred on all part D Medicare plans I've looked at?

Specifically: Sucralfate oral suspension. The liquid version has been generic since 2019.

There is a pill version that is so huge it's impossible for my husband to swallow after his esophagectomy.
 
@chrisboon I have a generic medication that runs around $500 a month. It used to be that generic was less expensive but that went out the window some time ago.

A lot depends on a competitive market. If only one company is making the drug then it's going to be expensive even if it's generic.
