Proof of Loss Form

For starters, I am in Sacramento, California. I’ll make this as brief as possible, I volunteer for a coroners office and got a very minuscule injury. It’s not something that I’m interested in seeking medical attention for or making any kind of claim. Today they gave me a proof of loss accident claim form to fill out and I’m just confused about why I’m filling it out if I’m not making a claim. Do I have to fill this out and return it? Is this beneficial or detrimental to me? Thanks in advance!
 
@callingantioch They probably assume you want to file a claim for workers comp. Just talk to whoever gave you the form and be up front about your intentions and they will point you in the right direction
