How do I pick a plan? (UHC)

I am switching jobs that had an amazing 100% company-contributed plan to a job with dog sh*t plans (but I'll be making $25k more a year so hopefully that offsets it?)

UHC Plan 1

Premium: $408.00
Deductible : $2,000.00
Max OOP: $7,000.00

Co-pay: 0% Co-insurance after deductible

UHC Plan 2

Premium:$206.00
Deductible: $4,000.00

Max OOP: $9,100.00

Co-pay: $40, deductible does not apply

UHC Plan 3

Premium: $271.00
Deductible: $4,250.00

Max OOP : $7,300.00
Co-pay: 0% Co-insurance after deductible

Female, 35
-Doctor's Appointments (outside of physicals): 3-4 times a year

-Therapy once a week

-On 2 prescriptions that won't be covered
 
@hiphopopotomus One thing to keep in mind when picking a plan is to always know what the max out of pocket is. That is the absolute max that you can come out of your pocket in a calendar year when it comes to medical bills.

Plan 2: Annual Premium $2,472 + $9,100 OOP max = $11,572 total possible annual cost ($4k ded)

Plan 3: Annual Premium $3,252 + $7,300 OOP max = $10,552 total possible annual cost ($4,250 ded)

I'm assuming Plan 2 is a PPO, what about the others? Do any of the plans have an HSA attached?

Note: Co-pays typically do not go toward the deductible.
