    @jackcv The provider agrees to the contracted rate when they join your insurance network, they cannot bill you for more than what your EOB states is your responsibility. Contirm this with your dentist and call your insurance back if they don't budge.

    @hermana EPO (exclusive provider organization) functions like a PPO (preferred provider) in that you don't need referrals to see specialists and will have a broader network, but has HMO (health maintenance organization) benefits where HMOs don't cover out of network care.

    Prior authorization after rescheduled procedure?

    @peacelovehope You should be fine. If the procedure fell outside the date range range, retro-authorizations are super common.
